How will I get my funds?

Eat Well offers $300 per month for up to 3 months (12 weeks) for eligible SFHP members to purchase fruits, vegetables, and beans.

Once you are authorized to enroll, you will get an Eat Well card in the mail. The card is sent by Eat Well and takes about 10-20 days to arrive. It comes with instructions to activate the card by phone or online.

Once the card is activated, you will have access to $300 per calendar month for 3 months, for a total of $900 to spend. You will receive your first month’s benefits on the day that you activate your card. The remaining benefits will be put on the card on the first day of the month for the next 2 months.

Funds roll over from month to month and are available until the end of your authorization period, when the card expires.

What can I buy?

Your Eat Well card can be used to buy fresh, frozen, or canned fruits, vegetables, and beans only. It does NOT cover any other healthy foods, such as meat, dairy, eggs, or whole grains.

To see if a specific item is eligible, download the OTC Network app, enter your card number, and scan bar codes at the store.

Buy what you want, when you want! You choose the fruits, vegetables, and beans you want to buy. You can spend your funds on multiple trips to the grocery store.

Where can I shop?

Participating Eat Well stores in the San Francisco area include Safeway, FoodsCo, Andronico’s, Costco, H Mart, Walmart, Heart of the City Farmers Market, Walgreens, CVS, and Dollar Tree, as well as some independent grocery stores.

You can also shop online at Instacart, Amazon Fresh, and Walmart.

Note: The Eat Well card cannot be used to pay for fees associated with online ordering or delivery. To redeem Eat Well benefits through online retailers, you will need to enter both an Eat Well card number and a personal credit or debit card as payment methods.

how do i use my card?

Easy! When you check out, swipe your Eat Well card like a credit card. The cost of eligible fruits, vegetables, and beans will be taken off your total. Then pay for the rest of your groceries with any payment you choose.

Note: You do not need to show your Eat Well card to the cashier, or enter a PIN. Make sure to use the card like a credit card. Do not choose the debit card option, the card will decline.

how do i check my balance?

You can check your Eat Well card balance by calling 1-888-682-2400, using the OTC Network app, or visiting www.mybenefitscenter.com.

What is the OTC Network app?

One way to check if an item is covered by the Eat Well card is to scan the barcode using the OTC Network app while you’re in the store. The OTC Network app can also be used to check your balance, view transactions, and find participating stores nearby.

The OTC Network app can be downloaded from your phone’s app store. To use the app, enter your card number (all 19 digits) and enter your birthday in this format: MMDDYY. Enter only the last two digits of the year.

Note: You do not have to use the OTC Network app to use the Eat Well card. It is an optional tool to use if you want!

Am I eligible for support with eating healthy?

Yes! All SFHP members authorized for Eat Well produce prescription card vouchers are authorized to receive nutritional counseling services from a Registered Dietitian.

Meals

How will I get my meals?

Eat Well provides eligible SFHP members with prepared healthy meals delivered directly to your home address. We cannot deliver to P.O. boxes. 

Once you are authorized and have completed a nutritional assessment with a Registered Dietitian, you will receive one meal box each week for 12 weeks.

What is in my meal box?

Each box contains a total of 14 refrigerated (not frozen) meals for the week, 2 meals per day.

The menu is pre-selected based on the dietary needs provided by your health plan. Our healthy meals are created by chefs and approved by dietitians.

Meals are delivered in a climate-controlled box and never frozen. Meals are to be refrigerated and can be heated and ready to eat in just two minutes.

Meals include lean meats, healthy grains, and vegetables. We can accommodate most dietary restrictions and allergies.

Am I eligible for support with eating healthy?

Yes! All SFHP members authorized for Eat Well healthy prepared meals are required to completed a nutritional assessment with a Registered Dietitian before meal delivery can begin.

Who is eligible to receive services from Eat Well?

  • Must be a Medi-Cal SFHP member with a chronic medical condition and with extensive coordinated needs or at risk of hospitalization.

  • Must live within San Francisco county.

How can I submit a referral to Eat Well?

Care Managers/Providers can refer members to Eat Well, or members can refer themselves.

To submit a referral, please visit the the Eat Well online referral portal. To submit the referral, you will need to download, complete, and attach the SFHP Eat Well Referral Form within the online portal, as well as medical documentation of a chronic medical condition.

What will happen after i submit a referral?

SFHP will determine if the member meets eligibility criteria and approve or decline referral. If approved, a member of the Eat Well team will contact the authorized member to confirm their address, any dietary needs (if receiving meals), and to complete enrollment.
