How will I get my meals?
Eat Well Rx provides eligible UHA members with prepared healthy meals delivered directly to your home address. We cannot deliver to P.O. boxes.
You will receive one meal box each week for 24 weeks.
What is in my meal box?
Each box contains a total of 21 refrigerated (not frozen) meals for the week, 3 meals per day.
The menu is pre-selected based on the dietary needs provided by your health plan. Our healthy meals are created by chefs and approved by dietitians. Meals are delivered in a climate-controlled box and never frozen. Meals are to be refrigerated and can be heated and ready to eat in just two minutes.
Meals include lean meats, healthy grains, and vegetables. We can accommodate most dietary restrictions and allergies.

How will I get my pantry stocking box?
Eat Well Rx provides eligible UHA members with healthy pantry stocking boxes delivered directly to your home address. We cannot deliver to P.O. boxes.
You will receive one grocery box each week for 24 weeks.
What is in my pantry stocking box?
Pantry stocking boxes include fresh fruits and vegetables, beans/legumes, healthy grains, dairy products, protein including lean meats, poultry, and eggs, as well as shelf-stable goods like healthy oils, spices, and herbs.
We can accommodate vegetarian/vegan diets as well as nine allergens: eggs, fish, milk, peanuts, shellfish, sesame, soybeans, tree nuts, and wheat.

Fruit and vegetable boxes are smaller than our pantry stocking boxes and include fresh fruits and vegetables only.

Referrals
Who is eligible to receive services from Eat Well Rx?
Must be an Oregon Health Plan UHA member with a health condition that would be improved with nutrition support, and at least one of the the following applies to you:
You left a mental health or substance use treatment program in the past year.
You left jail, prison or another correctional setting in the past year.
You have ever been in foster care or another child welfare program.
You are moving from Medicaid to both Medicaid and Medicare. This can be within the next three months or during the past nine months.
You are at risk of losing your housing or do not have a safe place to live.
You are 19 to 26 years old and have special health care needs.

How can I submit a referral to Eat Well Rx?
Care Managers/Providers can refer patients to Eat Well Rx, or members can refer themselves.
To submit the referral, you will need to download, complete, and attach the UHA Nutrition Services Request Form within the online portal, as well as medical documentation of a chronic medical condition.
What will happen after I submit a referral?
UHA will determine if the member meets eligibility criteria and approve or decline referral. If approved, a member of the Eat Well Rx team will contact the authorized member to confirm their address, any dietary needs, and to complete enrollment.
