How will I get my meals?
Eat Well provides eligible HPSJ members with prepared healthy meals delivered directly to your home address. We cannot deliver to P.O. boxes.
You will receive one meal box each week for 12 weeks.
What is in my meal box?
Each box contains a total of 14 refrigerated (not frozen) meals for the week, 2 meals per day.
The menu is pre-selected based on the dietary needs provided by your health plan. Our healthy meals are created by chefs and approved by dietitians. Meals are delivered in a climate-controlled box and never frozen. Meals are to be refrigerated and can be heated and ready to eat in just two minutes.
Meals include lean meats, healthy grains, and vegetables. We can accommodate most dietary restrictions and allergies.
Am I eligible for support with eating healthy?
Yes! All HPSJ members authorized for Eat Well prepared healthy meals are authorized to receive nutritional counseling services from a Registered Dietitian.

How will I get my grocery box?

What is in my grocery box?
Healthy grocery boxes include fresh fruits and vegetables, beans/legumes, healthy grains, dairy products, protein including lean meats, poultry, and eggs, as well as shelf-stable goods like healthy oils, spices, and herbs.
We can accommodate vegetarian/vegan diets as well as nine allergens: eggs, fish, milk, peanuts, shellfish, sesame, soybeans, tree nuts, and wheat.
Am I eligible for support with eating healthy?

Who is eligible to receive services from Eat Well?
Must be a Medi-Cal HPSJ member with a chronic medical condition and with extensive coordinated needs or at risk of hospitalization.
Must live within San Joaquin, Stanislaus, Alpine, or El Dorado counties.
How can I submit a referral to Eat Well?
Care Managers/Providers can refer patients to Eat Well, or members can refer themselves.
To make a referral, please visit the the Eat Well online referral portal. To submit the referral, you will need to download, complete, and attach the HPSJ Eat Well Referral Form within the online portal, as well as medical documentation of a chronic medical condition.
What will happen after i submit a referral?
HPSJ will determine if the member meets eligibility criteria and approve or decline referral. If approved, a member of the Eat Well team will contact the authorized member to confirm their address, any dietary needs, and to complete enrollment.
