Eligible health plan of san joaquin (HPSJ) members in San Joaquin, Stanislaus, Alpine, and El Dorado counties can now receive healthy Grocery boxes or medicailly Tailored Meals from Eat Well.

Eat Well offers healthy food boxes and healthy prepared meals to HPSJ Medi-Cal members with chronic health conditions.
This service is available to eligible HPSJ members with chronic medical conditions
AND ONE OF THE FOLLOWING:
1. At high risk of hospitalization or nursing facility placement
2. Requires extensive care coordination
3. Individuals with extensive care coordination needs.
Healthy food boxes or healthy prepared meals are delivered via mail directly to the participant’s home in a temperature controlled box. The food box and meals reflect the participant’s dietary needs and preferences providing access to healthy and primarily organic foods.
Grocery Boxes: Members will receive one grocery box per week containing 2/3 of a member’s nutritional needs, for 12 weeks. These services can be renew at the discretion of the health plan.
Healthy Prepared Meals: Members will receive one box a week containing 14 individually packaged and refrigerated meals that are ready to heat and serve for 12 weeks. These services can be renew at the discretion of the health plan.

Who is eligible to receive healthy food boxes from Eat Well?
Must be a Medi-Cal HPSJ member with a chronic medical condition at with extensive coordinated needs or at risk of hospitalization.
Must live within San Joaquin, Stanislaus, Alpine, or El Dorado counties.
How can I receive healthy food boxes from Eat Well?
Yourself or care manager/provider can refer you using the HPSJ Referral Form. Fill out the form along with medical documentation that shows your medical diagnosis and email it to referral@eatwellrx.org.
HPSJ will determine if you meet eligibility criteria and approve or decline referral. If approved, a member of our team will reach out to conduct initial assessment. Once enrolled, you will receive healthy food boxes from Eat Well tailored to your nutritional needs.
How do I get my healthy food box from Eat Well?
Boxes are delivered straight to your door, using on track or another national delivery service.
How often do I get a healthy food box from Eat Well?
Healthy food boxes are delivered once per week.
What if I’m having trouble with my food box delivery service?
If you have any issues, please contact us and our customer service team will assist you.
How long does the program last?
You will receive food boxes for up to 12 weeks with option to renew upon HPSJ discretion.
