Eligible health plan of san joaquin (HPSJ) members in San Joaquin, Stanislaus, Alpine, and El Dorado counties can now receive healthy Grocery boxes or medicailly Tailored Meals from Eat Well.

 

Eat Well

Eat Well offers healthy food boxes and meals to HPSJ Medi-Cal members with chronic health conditions.

This service is available to eligible HPSJ members with chronic conditions, such as but not limited to:

a) Diabetes, b) Cardiovascular Disorder, c) Congestive Heart Failure, d) Stroke, e) Chronic Lung Disorder, f) Human Immunodeficiency Virus (HIV), g) Cancer, h) Gestational Diabetes, i) High-Risk Perinatal Condition, j) Chronic or Disabling Mental/Behavioral Health Disorders. The diagnosis must be verifiable through the member’s medical records.

AND ONE Of THE FOLLOWING:

1. At high risk of hospitalization or nursing facility placement

2. Requires extensive care coordination

3. Individuals with extensive care coordination needs.

Healthy food boxes or meals are delivered via mail directly to a patient’s home in a tempature controlled box and reflect a patient’s dietary needs, as well as take into account member’s preferences. Providing Patients access to healthy & primarily organic foods.

Grocery Boxes:

  • Members will receive one grocery box per week containing 2/3 of a memebers nutritional needs, for a period of 12 weeks with the option to renew at discreation of Health Plan.

Meals:

  • Members will receive one box per week for a period of 12 weeks each box contains 14 refridgerated meals which are individually packaged, prepared, and ready to heat and serve. Option to renew at discreation of Health Plan.

Who is eligible to receive healthy food boxes from Eat Well?

  • Must be a Medi-Cal HPSJ member with a one or more of the chronic health care conditions listed above.

  • Must live within San Joaquin, Stanislaus, Alpine, or El Dorado counties.

  • Must have at least one physical or behavioral health criteria:

    • Patients with two or more qualifying chronic health care conditions.

    • Unhoused and/or housing insecure

How can I receive healthy food boxes from Eat Well?

Yourself or care manager/provider can refer using the HPSJ Referral Form. Fill out the form and email it to referral@eatwellrx.org. HPSJ will determine if you meet eligibility criteria and approve or decline referral. If approved, a member of our team will reach out to conduct initial assessment. Once enrolled, you will receive healthy food boxes from Eat Well tailored to your nutritional needs.

How do I get my healthy food box from Eat Well?

Boxes are delivered straight to your door, using on track or another national delivery service.

How often do I get a healthy food box from Eat Well?

Healthy food boxes are delivered once per week.

What if I’m having trouble with my food box delivery service?

How long does the program last?

You will receive food boxes for up to 12 weeks with option to renew upon HPSJ discretion.
