Eat Well offers healthy food boxes to CCAH Medi-Cal members with chronic health conditions.
This service is available to eligible CCAH members with chronic medical conditions
AND ONE OF THE FOLLOWING:
1. At high risk of hospitalization or nursing facility placement
2. Requires extensive care coordination
3. Individuals with extensive care coordination needs.
Healthy food boxes are delivered via mail directly to the participant’s home in a temperature controlled box. The food box reflect the participant’s dietary needs and preferences providing access to healthy and primarily organic foods.
Members will receive one grocery box per week containing 2/3 of a member’s nutritional needs, for 12 weeks. These services can be renew at the discretion of the health plan.

Must be a Medi-Cal CCAH member with a documented chronic health condition
Must live within Santa Cruz, San Benito, Monterey, Mariposa or Merced counties.
How can I receive healthy food boxes from Eat Well?
Yourself or care manager/provider can submit your referral request by submitting your CCAH information along with medical documentation showing your medical diagnosis to us. You can email us these documents at referral@eatwellrx.org.
CCAH will determine if you meet eligibility criteria and approve or decline referral. If approved, a member of our team will reach out to conduct initial assessment. Once enrolled, you will receive healthy food boxes from Eat Well tailored to your nutritional needs.
How do I get my healthy food box from Eat Well?
Boxes are delivered straight to your door, using FedEx or another national delivery service.
How often do I get a healthy food box from Eat Well?
Healthy food boxes are delivered weekly.
What if I’m having trouble with my food box delivery service?

How long does the program last?
You will receive food boxes for up to 12 weeks with option to renew upon CCAH discretion.
