Eat Well offers healthy food boxes to enrollees. Your care manager will specify how many food boxes you are eligible to receive.

Healthy food boxes are delivered via FedEx directly to a patient’s home.

  • Boxes can reflect patient preferences.

  • Patients have access to healthy recipes using box ingredients.

  • Patients receive one box per week for as long as they are eligible.

Who is eligible to receive healthy food boxes from Eat Well?

  • Must be a Medicaid managed care member enrolled in Standard Plan or enrolled in I/DD Tailored Plan.

  • Must live in pilot region.

  • Must have at least one physical or behavioral health criteria:

    • Adults with two or more qualifying chronic conditions.

    • Pregnant women with high risk of complications.

    • Child under 3 years old who graduated from neonatal intensive care unit.

    • Child under 20 years old experiencing three or more categories of adverse childhood experiences.

  • In addition to one physical or behavioral health criteria, must have one social risk factor:

    • Unhoused and/or housing insecure

    • Food insecure

    • Transportation insecure

    • At risk of, witnessing, or experiencing interpersonal violence


How can I receive healthy food boxes from Eat Well?

Your care manager will determine if you meet eligibility criteria and will assist you with enrollment. If you think you may be eligible, call the phone number on the back of your insurance card and let them know you’d like to join the Healthy Opportunities Pilot. Once enrolled, you can request a healthy food box from Eat Well.


How do I get my healthy food box from Eat Well?

Boxes are delivered straight to your door, using FedEx or another national delivery service.


How often do I get a healthy food box from Eat Well?

Healthy food boxes are delivered weekly.

How long does the program last?

You will receive food boxes for as long as your prescription indicates.
